Licensing authority
- •Authority name, phone number, and email
- •Your base/town of work
Documents (with expiry dates and clear photos of each)
- •DVLA electronic check code - a screenshot from the GOV.UK View Driving Licence service
- •Driving licence - front and back photos
- •Hackney/PHV licence - paper and badge photos
- •Vehicle tax - a screenshot from the GOV.UK Check Vehicle Tax service
- •Operator's licence - number, expiry date, and photo
- •Hire & reward insurance - provider, policy number, expiry date, and certificate photo
- •MOT certificate - expiry date and photo
